Plagerine 75mg is an antiplatelet medication (often called a “blood thinner”). It is primarily used to prevent serious cardiovascular events, such as heart attacks and strokes, in people who are at high risk. It works by preventing platelets in the blood from sticking together and forming dangerous clots that can block blood flow in the arteries.
The active ingredient, Clopidogrel, is a prodrug that must be activated by liver enzymes (specifically CYP2C19) to work. It functions by selectively and irreversibly inhibiting the binding of adenosine diphosphate (ADP) to its platelet receptor. By doing so, it stops the activation of the glycoprotein IIb/IIIa complex, which is the final pathway for platelet aggregation. This effect lasts for the entire lifespan of the platelet (about 7–10 days). It is frequently prescribed to patients with recent heart attacks, strokes, peripheral arterial disease, or those who have recently had a stent placed in their heart.
Clinical management of Plagerine requires awareness of an increased bleeding risk. Even minor injuries may take longer than usual to stop bleeding. It is strictly contraindicated in patients with active pathological bleeding, such as a peptic ulcer or intracranial hemorrhage. Patients should avoid taking NSAIDs (like Ibuprofen or Naproxen) while on Plagerine, as this further increases the risk of stomach bleeding. Additionally, certain “proton pump inhibitors” (like Omeprazole) may reduce its effectiveness by interfering with the liver enzymes needed to activate the drug. Common side effects include bruising, nosebleeds, and gastrointestinal upset.
